Meeting Registration & Hotel Lodging Requests
To complete the online registration form/hotel request, click here. This will take you to our Online Registration Form, which is being hosted by offsite by SysLab, an information services company working with PEER.
If you have already registered but need to make changes to your reservation/lodging request, please do not re-register for the meeting. Instead, please simply send an email to PEER Headquarters with any changes you may have to your lodging reservation or meeting registration. A confirmation email will be sent to you to let you know that we received your request/changes.
Fill the form out electronically
Open the form in Adobe Acrobat and enter your information electronically by using the Hand tool to select the different fields on the form. Once your data is entered, there are 2 ways to proceed:
a. save the document (the information will not be retained on the form unless the file is saved) and email it to: firstname.lastname@example.org. You will need the full version of Acrobat in order to use this option.
b. print the document and fax it to the PEER Outreach Office at 510-231-5669.

Print the form and fill it in either by hand or typewriter. Once your information is entered, fax it to the PEER Outreach Office at 510-231-5669 or mail it to: PEER Annual Meeting, 1301 S. 46th Street, Richmond, CA 94804-4698.
